Why your weight loss medication isn’t working anymore: Understanding weight loss plateaus

If your weight loss has slowed down or stopped while taking medications like Ozempic or Mounjaro, it can feel really frustrating. You may start wondering whether the medication has stopped working, or whether you are doing something wrong.
The good news is that this is very common. In many cases, it simply means your body has adapted and your plan needs a fresh look.
What is a weight loss plateau?
A weight loss plateau is when your progress slows down or stops, even though you are still taking your medication and trying to stay on track. It can be demotivating, but it is often a normal part of the process.
Why does weight loss slow down on medication?
There are a few reasons this can happen. As your body changes, it may become more efficient, your hunger signals may shift, and your metabolism may respond differently over time.
Your body is adapting
As you lose weight, your body naturally burns fewer calories than it did before. It also starts trying to protect what it sees as your new ‘normal’ weight, which can make progress slower.
Your hormones are changing
Weight loss affects the hormones that help control hunger and fullness. Even when medication is helping, these shifts can still influence how you feel and how your body responds.
You may be losing muscle too
If you are losing muscle as well as fat, your metabolism can slow down further. That can make it harder to keep losing weight at the same pace.
Your plan may need adjusting
What worked well in the beginning may not be enough at later stages. Weight loss treatment often needs to be adjusted as your body changes.
Does this mean the medication has stopped working?
Not always. In many cases, the medication is still doing something helpful, but other factors may be making progress slower. That is why a proper review is often more useful than simply assuming the dose needs to go up.
Should the dose be increased?
That is a common question, but a higher dose is not always the answer. Sometimes the plateau is happening because of several things at once, and it needs a more personalised approach.
How can a bariatric specialist help?
A specialist can look at the bigger picture and help work out what is affecting your progress. Instead of guessing, you get a clearer idea of what is happening and what might help next.
Understanding your metabolism
We look at your overall health, medical history, and any factors that may be slowing progress.
Fine-tuning your nutrition
Sometimes small changes, especially around protein intake, can make a real difference in protecting muscle and supporting fat loss.

Looking beyond the scale
The number on the scale does not always tell the full story. Body composition analysis can help show whether you are losing fat, muscle, or both.
Personalising your plan
Your medication, dosage, and follow-up schedule should be tailored to you, not based on a one-size-fits-all template.

When should you book a review?
It may be time to check in with a specialist if:
- Your weight has not changed for a few weeks.
- You are doing everything you can, but results have slowed down.
- You feel stuck or unsure about what to do next.
Sometimes, small adjustments are all it takes to get things moving again.
